To request your 1095-C form from EnterpriseDB, please email hr@enterprisedb.com stating your full name and the tax year for which you need the document.
While this form provides important details about your health insurance coverage, please keep in mind that you are not required to have it in hand to file your taxes. The IRS primarily uses the 1095-C for record-keeping purposes to confirm you had "minimum essential coverage," and you do not need to attach it to your tax return.
